The most common problem with this script is the .htaccess file. The .htaccess file needs to be in your root directory. Also, if you are running a VPS or VDS you must make sure that the httpd.conf file allows .htaccess files to override it ("allowoverride all" etc). If this is not set up correctly it will lead to 404 errors.
Most shared hosting providers allow .htaccess files to override directives set in the configuration. You should first test your provider to see if this overriding works before installing the proxy
